Vendor Request Submittal
Any company interested in becoming a supplier vendor of Triangle Three Resources, for consideration please fill out and submit the following attached a pdf. You’ll find a Master Service Agreement, Worker’s Comp Waiver, and W-9. The last page is an example of a certificate of insurance. Please have your insurance company forward us a copy of yours, preferably with the wavier of subrogation, as shown. Please mail the executed original documents (care of Kelley Crail) at your convenience.
